Note: If you choose to "Open Account" with us for Credit, and wish to process an order immediately while waiting for the credit application, you may call us after submitting your application and provide a credit card over the phone for use on the single order.

To open an account with your credit card please finish the form. Once completed and submitted we will email you a PDF for security of your Credit Card number. Print, and fill out your Credit Card Number on this form, then fax to us at 419-594-2250, or simply call us at 1-800-553-1911 to provide Credit Card Details over the phone.

OPEN ACCOUNT TERMS: NET 15 DAYS

Please note: Net 15 days are our terms for an open account.

There will be a 2% service charge on accounts over 30 days. No orders will be shipped with the invoice over 30 days of age, or unpaid finance charges.

There must be credit approval before merchandise can be sent on an open account. New accounts will be shipped C.O.D. until credit is established. We accept American Express, Discover, VISA and Mastercard for payment.

TERMS OF SALE

EFFECTIVE JANUARY 31, 2012 • SUPERCEDES ALL PREVIOUS LITERATURE

PRICES: Prices are subject to change without notice and all merchandise will be shipped at prices in effect at the time of shipping.

TERMS: All new accounts are C.O.D., Visa, MasterCard or cash advance. New dealers must comply with a minimum order of $50.00 unless the dealer is compliant under our Dealer Support Guidelines. Companies with 2 years prior storefront business may be placed on Net 15 terms after a rigid credit review, but are still subject to the above minimum order requirement. Businesses with no prior business history must pay by cash advances, C.O.D., Visa or MasterCard. Any account over 30 days past due will be charged a 2% per month finance charge and all orders will be placed on hold. Delinquent accounts will be returned to C.O.D. If an account is sent to collections, all cost on your account must be paid in full before any orders will be accepted, and then on C.O.D., Visa or MasterCard basis for all future orders.

C.O.D.s: C.O.D.’s that are returned, due to unavailability of a person or funds, will be paid by the customer, along with any and all shipping charges. Any orders refused by a customer will require payment in full before any future orders are filled.

NSF CHECK: Any account having a check returned from the bank for “Not Sufficient Funds” will be charged $35.00 for the “Returned Check” and placed on C.O.D. money order or certified check basis. If a returned check is not satisfied (amount paid to cover check) we will prosecute to the full extent of the law, including ALL collection costs.

HANDLING CHARGES: All orders less than $50.00 are subject to a $3.00 handling charge unless dealer is compliant with our Dealer Support Program. All drop shipments, including those on customer shipper numbers, will incur a $2.00 fee and will also be charged at the residential rate unless the dealer is part of our Dealer Support Program.

SAMPLE REQUESTS: Sample requests will be billed at the unit price and will be shipped C.O.D. unless terms have been established with our finance department. The item must be returned using the listed return policy to receive credit. All samples must be returned clean and in resalable condition. Freight will not be credited.

RETURNS: Vest and special orders are non-returnable. All claims for returns must be made within 30 days of shipment to avoid restocking fees. Returns made up to 60 days will have a 20% restocking fee. Returns after 60 days and up to 90 days will have 30% restocking fee. No returns after 90 days. Freight and other charges will not be credited. Returns will be refused unless a return authorization number has been issued. We guarantee the material and workmanship of all products manufactured by R & B Fabrications, Inc.

REFUSED SHIPMENTS: Refused shipments are subject to freight charge backs and 20% restocking fees.

FREIGHT: All shipments are FOB Oakwood, OH. We assume no liability for delayed deliveries by UPS or any other carrier if the delays are beyond our control. Any charge backs from UPS or any other carrier will be charged back to the dealer.

CLAIMS: Claims for shortages, flaws or incorrect merchandise must be made to R&B Fabrications, Inc. within 10 days. Deductions from remittances will not be allowed unless authorized by us in writing. All claims for goods lost or damaged in transit must be filed by the customer with us. All packing material and paperwork must be held for inspection.

DYE LOTS: We are not responsible for dye lot variations.

ERRORS: We are not responsible for errors on custom work due to orders that are not written clearly, or for orders taken over the phone or placed verbally.

LICENSE RIGHTS: All artwork is custom designed for our customers. We hold NO license rights, except as noted, and accept no liability for copyright infringement for any design.
